A free-zone licence changes who processes your employee's paperwork. It does not change the health insurance duty behind it. Does a Free-Zone Licence Change the Health Insurance Duty?
No. The obligation to insure an employee follows the sponsorship, and the health standard follows the emirate that issues the residence visa. A Dubai free-zone company sponsoring an employee in Dubai has to meet the Dubai floor, including the Essential Benefits Plan package where the salary threshold applies.
What the licence does change is the counterparty. Mainland employers deal with the Ministry of Human Resources and Emiratisation for permits; free-zone employers deal with their zone authority, which runs its own portal, its own document set and its own timelines on top of the federal and emirate requirements.
What Actually Differs Between a Mainland and a Free-Zone Process?
- Who issues the permit. The zone authority handles establishment cards, permits and visa quotas for its own companies rather than MoHRE.
- Which portal you submit through. Each zone runs its own, and the order of steps — entry permit, medical, Emirates ID, stamping — is sequenced by the zone.
- What the checklist asks for. Zones commonly require proof of insurance at a specific step; ask which step, because arranging cover a week late blocks the file rather than the person.
- Who chases what. Many zones work through approved service partners or a PRO, which adds a party between the employer and the authority.
- What is identical. The health cover standard, the employer's duty to pay for it, and the consequence of a lapse. None of these are zone-specific.
What About DIFC and ADGM?
The two financial free zones are a genuine exception in one narrow sense: they have their own legal and regulatory frameworks, including their own employment regimes and their own financial services regulators. For health insurance, though, the cover still has to satisfy the health authority of the surrounding emirate — the Dubai Health Authority for DIFC, the Department of Health for Abu Dhabi's ADGM — because that is who governs the health floor and the residence visa behind it.
Treat the financial free zones as a different employment rulebook sitting on top of the same health requirement, and confirm the current position with the zone's own employment guidance rather than assuming it mirrors the mainland.
Which Myths Cost Free-Zone SMEs the Most?
- "A free-zone licence makes medical cover optional." It does not. The duty follows sponsorship.
- "Any basic policy is fine, indefinitely." Minimum packages, networks and thresholds change. A plan that was compliant two renewals ago may not be now.
- "Free-zone plans use the same hospitals as mainland plans." Networks are set by the plan and the tier, not by the licence type. Check the list.
- "The cheapest quotation proves compliance." Compliance is a question about the benefit package and the authority's requirement, not about price.
- "The zone will tell us if something is wrong." The zone checks its own file. Whether the plan suits your workforce is nobody's job but yours.
What Should a Free-Zone SME Check Before Onboarding?
- Headcount now and the twelve-month hiring plan, so mid-term additions are priced rather than improvised.
- Where staff live and where they would actually seek care, which is what makes a network list good or useless.
- Whether dependants are included, and if not, that the employee has been told clearly and early.
- The zone's own document checklist, and the step at which it asks for proof of insurance.
- Who explains claims, approvals and network rules to a new joiner in week one. Someone has to, and it is usually HR.
How Should Free-Zone HR Run a Renewal?
Exactly as a mainland team does, on the same 90-day discipline: confirm the expiry date, rebuild the census from payroll, ask for the claims report and the loss ratio around 60 days out, get terms priced against the same benefit table, and have the policy issued before expiry rather than merely agreed. The zone's timelines sit alongside that calendar; they do not replace it. The renewal timeline has the checkpoints in order.
What Changes When You Hire Outside Dubai?
The emirate that issues the residence visa sets the standard, so a Dubai free-zone company sponsoring somebody in another emirate is working to that emirate's rulebook, not Dubai's. Staff sponsored in Abu Dhabi sit under the Department of Health; staff in the northern emirates route through the federal workers scheme. Ask the insurer to confirm, in writing, that the plan satisfies the requirement in every emirate where you sponsor people.
